The right candidate will be responsible for providing comprehensive care coordination and discharge planning for all patients, ensuring continuity of care and cost-effectiveness.

Key responsibilities will include:
• Quality Improvement: Developing and implementing patient care policies and protocols, conducting reviews to ensure appropriate resource utilization, and evaluating patient satisfaction.
• Collaboration: Working closely with physicians, social workers, and other healthcare professionals to create a patient-centered care plan.
• Leadership: Providing ongoing support and expertise, while promoting the organization’s mission, vision, and values.

Qualifications include:
• BSN required; MSN preferred
• RN licensure
• CCM or ACM certification
• Minimum 5 years of management experience, 5 years of utilization review/case management experience, five years experience in discharge planning in an acute care setting
